The team here consists of professionals who work together to provide the care our patients need. Here is a breakdown of the team and their roles.

Medical Director

The medical director leads the team. They oversee the medical care to make sure it fits with the patient's needs. Their primary role involves working with other healthcare providers listed below to manage pain, symptoms, and medications to provide the highest standard of care. The medical director guides the overall direction of the care plan and adjusts it where necessary, as the patient's needs may change over time,

Nurses

Nurses are at the core of hospice care. They provide daily care, check the patient's symptoms, and administer medication. Nurses work closely with the medical director to manage the pain and symptoms and ensure the patient's comfort. They also interact extensively with family members to update them on how the patient has been doing and offer emotional support.

Social Workers

Social workers help support patients and families to address emotional, social, and practical needs. Their varied role includes assisting with how the healthcare system works, helping access community resources, and setting up counseling. They also help communicate between hospice and the family so that everyone is kept up to date on what's going on.

Chaplains/Spiritual Counselors

Chaplains and spiritual counselors help with the needs of patients and their families to help respect. They help by offering comfort, guidance, and rituals that can help bring peace during this time. This can be done through prayer, conversation, or just generally being a supportive presence.

Home Health Aides

Home Health Aides help with personal tasks such as dressing, bathing, and grooming, which helps patients maintain their dignity and comfort. They are specially trained to care with respect for the patients' routines and preferences, which can help them stay in their homes surrounded by loved ones.

Volunteers

Volunteers play a big part in hospice care by offering companionship, practical help, and caring when no one is there. They can help with running errands or even reading to a patient.
